Submit the nomination form (click on above link to open) along with a one-page narrative describing why the person deserves to be recognized. Include who, what, and why together with any supporting information that will assist the awards committee in the selection process. Please limit supporting documentation to two additional pages.
Presentations will be made to the award recipients at Thursday's luncheon on April 26. Please respect the following nomination deadline for the Fall 2011 Conference:
- Spring 2012 Conference - Deadline is September March 30, 2012.
